This interactive session introduces a structured, end-to-end approach to analyzing inpatient encounters—from admission through discharge—while highlighting common documentation gaps that place organizations at risk for denials, downgrades, and recoupments.


Through facilitator-guided instruction and collaborative small-group case review, attendees will practice identifying insufficient or unclear documentation, evaluating diagnostic support, and understanding how documentation weaknesses translate into payer scrutiny.
